feat(provenance): add OpenQC traceability report - #150

Summary

• add deterministic OpenQC v1 docstring/wiki/raw traceability report generation
• link source docstrings/doc comments to concrete wiki nodes and wiki source nodes to raw evidence
• add focused traceability contract tests and generator check mode
